• A photocopy of your child’s birth certificate
• A photocopy of your child’s immunization card (including cover page and record page, clearly showing the child's name)
• A photocopy of your child’s passport (including those pages with valid entry permit) (For applicants without HK birth certificate only
• A photocopy of your child’s report(s) from the current and previous school(s)
